The type of personal information we collect

We collect and process information including but not limited to:
  • Personal information, such as name, date of birth and contact details
  • Information about your health including medical history, allergies, medication you may be/have been on, results of any medical investigations, treatments etc
  • Financial details with regards to payments e.g. bank details and receipts

Change of Details:

It is of utmost importance that you let the Clinic know in case of any changes that need to be made to your personal information (e.g. a change in address) or if you find any incorrect piece of information in the data that we have collected, that may need amending (e.g. your phone number, date of birth). You have a responsibility to inform us of any changes so our records are accurate and up to date.

What are Cookies?

Cookies are text files containing small amounts of information. Vitamin Suite uses two types of cookies:

The clinic has a legal obligation to maintain your confidentiality and is committed in doing so. All the staff is trained in keeping your personal data secure and confidential in line with the guidelines set out by the GDPR

How we store your personal information

Your paper medical records are securely stored in locked filing drawers. Electronic files are saved in password-protected computers.

Your information will only be passed on if ever a genuine need for it arises. The Clinic will never disclose your information to any third party without your permission, unless there are exceptional circumstances; e.g. if it’s a life or death situation, or where it is our legal obligation to pass on any information as part of our duty of care.

Vitamin Suite will follow standard medical guidance on the length of retention of medical records. In most cases, this means that your records be retained for 8 years after you last saw us. It will then be disposed of in a safe manner.

Records may be kept for longer provided that there is good reason to extend the retention period (e.g. if your medical condition is the subject of a protracted legal claim). In the case of child patients, medical records are normally kept until the child reaches the age of 25 or 26 years.

Your data protection rights

Under data protection law, you have rights including:
  • Your right of access – You have the right to ask us for copies of your personal information.
  • Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
  • Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances.
  • Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal information in certain circumstances.
  • Your right to object to processing – You have the the right to object to the processing of your personal information in certain circumstances.
  • Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you. Please contact us at admin@vitaminsuite.co.uk if you wish to make a request.

Complaints

If you have any concerns about our use of your personal information, you can make a complaint to us at admin@vitaminsuitre.co.uk . You can also complain to the ICO if you are unhappy with how we have used your data.
